Creating a foundation for success: Overview

Families and caregivers help create a foundation to support a student’s transition to adulthood. This can begin in the early childhood years with families setting a vision for a good life that is based on maximizing strengths and encouraging growth experiences.

As a student ages, families can establish high expectations for skill building that leads self-determination and real employment after high school. Disability may impact individuals in different ways, but the goal of transition remains the same—helping youth set their own future goals and supporting them to reach those goals.

In this section, learn about creating a foundation for success:

Setting a vision for a good life

Part of creating a foundation for a successful transition to adulthood is to set a vision for a good life and to understand how current activities are impacting the ability to reach those goals. The Charting the LifeCourse life trajectory worksheet is an easy to use tool that helps individuals and their families map points of a “good life”, as well as areas to avoid.
