Complete the intake and eligibility process

LEARN: Develop your knowledge
Read the following to build your knowledge about completing intake and eligibility with a person on a waiver.
The intake and eligibility process is that same whether or not a person is on a waiver, with two exceptions:
- The person will likely be referred to VRS/SSB by a waiver case manager or a waiver employment service provider. To make the referral, they may use the E1MN Common Referral Form. It’s important during intake to ask the person if they give approval for VRS/SSB to communicate with the person who referred them to VRS/SSB and their other team members.
- The person will likely have several helpful files, such as an employment portfolio, that will inform development of the VRS/SSB employment plan. It’s important to ask the person if they have any files to share and talk through the best way to share those files with VRS/SSB (e.g., through My Vault, other method).
1. Read about VRS/SSB intake and eligibility determination in the “VRS/SSB process” section of the “Find” page of this toolkit (5 min)
2. Read about what to include with the referral in the “Referring directly to VRS/SSB” section of the “Find” page of this toolkit (5 min)
3. Read: Internal VR policy and guidance “Chapter 1: Qualifying for Services” material on intake and eligibility (available to VRS/SSB staff through staff SharePoint)
